Railey's Initial Assessment

Railey Edward Siccion Medina is the youngest of the three sibling's and the only boy in the family. His mother teaches and his father presently takes care of him at home.

He was first enrolled at Bataan Angel's as reported by the informants who happens to be his father and grandmother.

He was brought to Shunem with the family's concern of his delay in language development.


TESTING PROPER 

Railey arrived at school with his grandmother and father with his feeding bottle in his mouth. Upon entering the room, he ran towards the bookshelf causing his father to call his attention. The father was advised  let the child explore around. Railey is talking in a distinct accent while he roams around the room. He was later brought to the play room to begin his testing.



DEVELOPMENTAL AREAS

I. PHYSICAL DEVELOPMENT

A. Gross Motor

      Railey can walk forward independently. He can run and jump from a table and chair alternately. He can open cabinets, carry blocks and boxes containing toys. He moves fast and can shift positions from sitting to bending. He can also crawl under tables when needed and has no difficukties in doing such.


B. Fine Motor

     Railey can hold pieces of puzzles, marbles and pegs efficiently. He can twists and untwist caps of any size of containers. He can screw and unscrew bolts and nuts and transfer marbles into an empty plastic bottle one at a time. He does puzzles very well.








II. LANGUAGE DEVELOPMENT

A. Receptive Language

     Railey can follow verbal ques to sit down and stand up with gestural prompting. He also understands when advised to wipe his nose.


B. Expressive Language

     Railey can say the kind of toy he is requesting to play with. During the assessment; he would say "pink blocks", "ABCs", and "puzzle". He keeps on telling "the door" when he wanted to leave the room. He reaches grabs the assessors hand when he wanted something he could not reach but cries and shouts when requests were not granted.



III. COGNITIVE DEVELOPMENT

       Railey can identify the letters of the alphabets, numbers 1-10, numerals and number words, basic colors and shapes. He also names animals.



IV. SOCIAL AND EMOTIONAL DEVELOPMENT

       Railey smiles when he is tickled in his neck. He cries and shouts when requests are denied. He seeks attention by grabbing the assessor's hand. He lies his head on the assessor's lap, maybe because he was sleepy.



V. SELF-HELP SKILLS
  
     Railey can remove shoes and socks independently. He can wipe his face with a towel. He blows his nose on command. He can eat crackers and drinks on plastic bottles on his own. He independently got his feeding bottle when he wanted to drink milk.
